PRACTICE TRIAGE POLICY
Our practice policy is that if you are on our telephone triage list for a callback, for an urgent on the day concern, then we will make 2 attempts to phone you. If you are not accessible (phone signal weak or phone on silent) this means we will not be able to speak with you on the phone. If you do not hear from us in 5 hours please contact reception.
Our Reception team Speak to hundreds of people every day and respect your confidentiality completely. They may ask you to send a photograph of your concern (rash or new visible lump), which speeds up the process. They will ask you details of your concern in order that the triaging GP can see at a glance whether your situation needs more immediate attention.
Please understand that we can have hundreds of people to phone back on triage some days, and we have to prioritise those who are in great difficulty, for instance, those who are suicidal or having a physical emergency. Please ensure we have the most up to date (ideally mobile)telephone number for you. Being able to SMS you also makes a big difference to our communication. We try very hard to get to everyone in good time. On busy days there may be a wait.

The 111 phone service is a 24 hr 7 days a week triage and medical advice service where the importance of your condition can be assessed by their expert system.
If they feel you need to be seen in General Practice, your care will be transferred to us.
Please also use the online system for ordering your repeat prescription where possible either using the NHS App or SystmOnline.
SMS Text Message
We are currently updating our records by using a system called Accurx and Florey. You may receive a text message asking you to fill out a questionnaire, which would be really helpful in updating your medical record.
Florey works by allowing the Practice to send a patient a text which contains a unique link to a pre-made questionnaire. The patient can then fill it out on their phone (or any device with internet) and the response comes back to the Practice and straight to your medical record!
Your GP might send you a text asking you to complete a few questions via a link. This might be in relation to a review of your Asthma, for example. Or you might be asked to complete a short survey to collect some other data about you (alcohol consumption, BMI, smoking status) or send a photo.
Please don't worry if you do not have internet access to respond.
Public health guidance remains in place suggesting individuals should continue to wear a face covering in crowded and enclosed spaces, where you may come into contact with people you do not normally meet.
With current case rates in South Yorkshire remaining high the Directors of Public Health for Barnsley, Doncaster, Rotherham and Sheffield are advising that well fitted and correctly worn face masks have made a difference to transmission rates through the course of the pandemic and are therefore encouraging people to continue to wear them.
You will still be required to wear a face covering when using NHS services. We ask that patients continue to wear face coverings when visiting our practice. Hand sanitiser is also available for you to use whilst in practice and we ask that you continue to maintain a safe distance from other patients whilst in the waiting room.

AVENUE SITE RENOVATIONS
We are undertaking considerable renovations at The Avenue Site over the next few weeks. This will potentially affect the availability of appointments at this site. We would ask for your patience and understanding whilst these works are carried out.
PLEASE MAY WE POLITELY ASK THAT PATIENTS PARK IN THE SURROUNDING AREAS AND WALK TO THE JORDANTHORPE HEALTH CENTRE WHERE POSSIBLE. THE CAR PARK WILL ONLY BE OPEN FOR PATIENTS WHO HAVE DIFFICULTY WALKING.
The Practice now employs a team of Clinical Pharmacists who deal with changes in medication, medication queries, medication reviews and anything which is passed on by a GP to discuss with a patient. The role of the Clinical Pharmacist within Practices is widely used to help free up Doctor time to deal with more complex, urgent cases.
We now have 3 Social Prescribers at the Practice targeting different age groups. They can help with a range of social issues including Housing problems, accessing food, benefits, loneliness and much more. Our Social Prescribing Advisors can support patients to unpick matters affecting their wellbeing and achieve personal goals.
With links to the local community and resources, and time to spend with patients building an action plan which asks 'what matters to you?', our Social Prescribers can support you to lead a healthy and fulfilling life.
Peak Edge Social Prescribing is a Social Prescribing service which covers our Practice funded by Peak Edge PCN and managed by AGE UK, South Yorkshire Housing Association and Sheffield Futures. If you would like to know more about this, please contact the Practice.
Registering with the Practice
If you would like to register with the practice, please click here. Alternatively, you can visit either site to collect a copy of our registration form. Please do not ring the practice to register. Please can you ensure you obtain a months supply of any medication from your current practice prior to registering at our practice.
TYPE 1 and TYPE 2 OPT OUT PREFERENCE
The data held in your GP medical records is shared with other healthcare professionals for the purposes of your individual care. It is also shared with other organisations to support health and care planning and research.
If you do not want your personally identifiable patient data to be shared outside of your GP practice for purposes except your own care, you can register an opt-out with your GP practice. This is known as a Type 1 Opt-out.
Type 1 Opt-outs may be discontinued in the future. If this happens then they may be turned into a National Data Opt-out. Your GP practice will tell you if this is going to happen and if you need to do anything.
You can use the form above to:
Register a Type 1 Opt-out, for yourself or for a dependent (if you are the parent or legal guardian of the patient) (to Opt-out)
Withdraw an existing Type 1 Opt-out, for yourself or a dependent (if you are the parent or legal guardian of the patient) if you have changed your preference (Opt-in)
You can also update your preferences using the NHS App.
This decision will not affect individual care and you can change your choice at any time, using this form. This form, once completed, should be sent to your GP practice by email or post.

Our Practice premises have suitable access for people with disabilities and there are designated car parking spaces close to the surgery entrance. Toilet facilities enable wheelchair access and support for disabled people.
